there should never be a tie for a playoff spot - the correct way to determine seedings is Wins and Points For ONLY (using decimals). If you use that method, you'll potentially never have a tie. Head to Head should be the third tiebreaker & only as a last resort, not the 1st - that's where your confusion is coming in - you're doing it bassackwards.

3 teams tied for one playoff spot
Our tiebreaker rules are
1) head to head record
2) most points scored
3) and 4) don’t matter in this case
Team A is 0-2 vs Team B
Team A is 1-1 vs Team C
Team B is 1-1 vs Team C
In this case Team B is 2-0 vs Team A and should clearly be on top of the tie breaker.
My only thought was if Team C had to be above Team B and Team C couldn’t be above Team A this would cause a 3 way tie issue but it doesn’t.
Can anybody help or explain?
