Alcantara has been able to play catch for about two months now and is presumably increasing the distance and frequency of his throwing, but the Marlins haven't indicated when he might be ready to toss bullpen sessions. Regardless, after he underwent Tommy John surgery on October 6, 2023, the Marlins aren't counting on Alcantara contributing at any point in 2024. The Marlins' focus remains on getting the 2022 National League Cy Young Award winner back to 100 percent for the start of spring training next year.

Alcantara threw four scoreless innings in a promising rehab start Thursday with Triple-A Jacksonville, but his elbow and forearm were barking again in the aftermath and he's now officially been shut down with what's being called a right UCL sprain. The reigning NL Cy Young Award winner said it has yet to be determined whether the injury will require a surgical repair. He finishes with a 7-12 record, 4.14 ERA, 1.21 WHIP and 151:48 K:BB in 28 starts covering 184.2 innings.

Alcantara is headed to Miami to be evaluated this weekend by the Marlins' medical staff, and it's a safe bet that he is not going to be able to return before the end of the regular season. He tallied four strikeouts over four scoreless innings Thursday with Jacksonville while touching 98.3 mph with his fastball, but the right UCL sprain that sent him to the injured list Sept. 6 is still clearly an issue.

Alcantara put up an impressive stat line, but more importantly, he managed to throw 51 pitches while mixing in all of his offerings. He also topped out at 98.3 mph. The next step for Alcantara is unclear, though he could take one turn through the rotation before the regular season comes to a close.

If that outing goes smoothly, Alcantara could make it back for one final regular-season start and perhaps help the Marlins in their quest to claim an NL wild-card spot. Per Jordan McPherson of the Miami Herald, Alcantara made full use of his arsenal and indicated afterward the session that he was feeling good.
